I've been in the DME/HME business for about 25 years. Got in around 1992. Used to be great fun but as someone else has mentioned "it's not for the faint of heart". Reimbursements have been lowered to a point of extremely tight, if any, margins in most product categories. Oversight is rampant and ongoing, for good reason due to all the fraud and abuse. First, being approved as a Medicare provider will go a long way in the future; I've been one for the whole 25 years. It's your gateway into most private pay and Medicaid contracts, assuming the private payers you're seeking are accepting new companies. The road to becoming a Medicare provider has become significantly more arduous that it used to be...licensure, background checks, Accreditation, insurance requirements, site inspection, etc., etc. Figure a good 6-9 months before being approved, during which time you may, or may not, be able to conduct any business. Then you need a good biller and collector. You'll be constantly denied or your claims will be audited, sometimes before you're paid and sometimes afterward. The auditing process is draconian in some regards and there is a 4 step appeal process. Currently this process is taking several years, during which your reimbursement is held up or, if you've been paid, they may have the right to recover. While a clean Medicare claim is typically paid in 3-4 weeks, private pay claims can take months. You'll need exceptional software that will allow you to file most claims electronically, which will speed up the reimbursement process. There's much more to the DME/HME business and if I was considering getting in now I probably wouldn't. The road is strewn with companies that have gone out of business, especially in the last several years. Read up, do some research, get HME News (say you're a consultant), talk to current owners, etc. Then think about it very carefully, you're in for one heck of a roller-coaster ride.

Can someone please help clarify, since I am new to the DME field. For online stores selling retail products (such as retail wheelchairs/powerchairs), is the DME license and physical location still required?

I'm an independent rep with my own LCC. My business is at the point now, where I would like to get my own DME license instead of using other companies as my billing arms. I'm in the orthopedic, wound, and chronic pain spaces. Can anyone help me with how to start this process? Any help would be greatly appreciated

I have been involved in the DME arena and have found there are many options to get started but it is best to start with a supply company that offers full support.
Sometimes you can go through a number of suppliers before you find a good one.

After reading your post it sounds like to have a lot of experience in the DME field. Our company deals with a lot of Medicare and private Insurance patients in need of DME and RX products. We are currently dealing with several companies for our Processing and billing but are interested in obtaining a relationship with reputable company we can potentially to business with. IF you know of such company could you be so nice as to send us a referral.

I unfortunately lost my DME company in a divorce. It was started in 2007. I have been through the ringer. I am now trying to start my own DME company. I have researched and of course this will be my second time getting accredited.
My main issue is getting in network with insurance companies. Please let me know if you know of a person/company to help with this. So many of them I have talked to do not seem give you a direct answer or any assurance that they can get you in network. Thank you!!

I have tried for years to become in-network with UHC nationally. I currently have a contract in Florida with UHC,the problem is I receive referrals from all over the US and have to pass due to being out of network in the other states..

I actually know a seller in Texas that has a DME company that the Owner might be willing to sell for less. My estimated guess is around $60K. The company is in-network with Medicare,
